This contract is bound by and between Rebecca Stephens, herein called breeder and ______________ herein called buyer. I (buyer) have
purchased a (Sex) ______ Maltese puppy on the above date. I understand that I have a 72 (seventy-two) hour health guarantee on this
puppy.

I (buyer) have/am purchased/ing the above described puppy/dog for $________. The deposit of $_____, Deposit is non refundable if I
(buyer) decide that I do not want said puppy.  If I (buyer) have paid extra towards the balance of the puppy/dog, the breeder will refund the
difference of the payment at the time that thepuppy/dog is placed in a different home.

All puppies/dogs will be checked by a licensed veterinarian prior to going to their new homes. And will come with a Health Exam Check
Sheet signed and dated by a licensed veterinarian. If puppy/dog is found to be in poor health after I (buyer) has placed the deposit.
Breeder will refund my (buyer's) full deposit.

I (buyer) understand that I have 72 (seventy-two) hours to take above mentioned puppy  to a licensed veterinarian of my choice.

If I (buyer) take this  puppy to the veterinarian in the time frame stated above, and found by the licensed veterinarian to be in poor health by
fault of the breeder and not injuries or illness while in my (buyer's) care, I am entitled, upon first availability, a replacement puppy of equal
value. Breeder has 12 (twelve) months to provide replacement. I (buyer) must provide a written statement as proof of a health issue and
problem within 1 (one) week from my licensed veterinarian to breeder and breeders veterinarian.

I (buyer) have a 1 (one) year contract against congenital health conditions that are life threatening. If puppy/dog is diagnosed by a licensed
veterinarian within this time frame to be sick with a congenital disease, I (buyer) will receive a replacement puppy/dog of equal value.
Breeder has 12 (twelve) months to provide replacement. I (buyer) will be allowed to keep original puppy after verification from breeders vet
of ailment that is genetic. Buyer will be responsible for all shipping (nanny service) charges.

If the puppy/dog develops a umbilical hernia and it is superficial and non-life threatening and I (buyer) decide to have it repaired, I will do
so at my own expense. Breeder is not responsible for cost of the repair.

This puppy  is sold as pet quality only unless otherwise stated. The size, weight, bite and coat are not guaranteed.

If the puppy/dog dies within the 1 (one) year time frame, I (buyer) am responsible for having my licensed veterinarian perform an autopsy
on the puppy/dog to diagnose the cause of death, at my (buyer's) expense. If the puppy/dog is diagnosed to have died due to a genetic
defect breeder will provide a replacement puppy of equal value to the buyer. Breeder has 12 (twelve) months to provide the replacement.

If I (buyer) choose not to take the above puppy('s) to the veterinarian for an examination within the 72 (seventy-two) hour time frame, I
(buyer) understand that I forfeit all of my right to a replacement puppy.

I (buyer) will never chain the above mentioned puppy/dog out side or leave him/her out doors for any extended amount of time. I (buyer)
will provide adequate care and nutrition for the puppy/dog at all times. If I (buyer) am unable to care for the dog/puppy, I will return the
puppy/dog to breeder.

Puppy is sold with Limited AKC Registration. At no time will I (buyer) use puppy/dog for breeding purposes. I (buyer) will have said puppy
spayed/neutered by the time he reaches 1 (one) year of age and provide breeder with proof of alter from a licensed veterinarian to breeder.
After breeder receives proof of spay/neuter, breeder will provide Limited AKC Registration Papers.

At no time will I (buyer) have the puppy/dog put down or otherwise disposed of unless required by a licensed veterinarian for the health
and well being of the puppy/dog. Proof of illness must be provided by a licensed veterinarian to the breeder. I (buyer) will not leave the
puppy/dog at a kill shelter.

I (buyer) will not take said puppy/dog into any public establishment until he is fully vaccinated against diseases.

In the event that this contract is breached and a lawsuit arises, the winner of the suit is entitled to recover their expenses, including
reasonable attorneys fee’s. The laws of the State of Florida will govern, and any claims will be handled in court in the State of Florida and
in the county in which the breeder resides.
